[framework] cmd.exe will not spawn a shell

I know this is a well know issue, but anyway... Can anybody, please, explain why
(sometimes) calling directly cmd.exe will not spawn a shell, while executing
"start cmd /K" will work as a cham. The question is refering to some Metasploit
Framework modules...
